New Patient Information

Medication Refills

Please call your pharmacy for all prescription refills. This will help ensure that your medication will be refilled in a timely manner. Please allow up to 48 hours for the pharmacy and clinic to process your request. Urgent situations will be handled on an individual basis.

Prior to Your Appointment

If you are a new patient, you may find our new patient paperwork under Patient Forms. We encourage you to print and complete these forms prior to arriving for your office visit. We ask that new patients arrive 30 minutes before your scheduled appointment to allow time for the registration process. Patients who arrive late to a scheduled appointment may be asked to reschedule. If you are unable to keep your scheduled appointment, please call at least 24 hours in advance to avoid a no-show/cancellation fee.

Fee is based on scheduled appointment time length that was missed:

<30 minutes: $50

31-60 minutes: $100

60+ minutes: $150
